Sigma Center for Counseling

Following completion of care, the client will have a one-hour individual session with their primary counselor where a Discharge Plan is prepared and agreed to by both parties.  The counselor will have the option of asking the client to return to the Outpatient group one last time for closure before terminating treatment.
 

Reasons for Discharge

• Client will have demonstrated 30 consecutive days without the use of alcohol and/or unlawfully obtained drugs.
  
• Client will verbalize to their counselor a commitment to improve the quality of life.
  
• Client will have completed 90% of all goals and objectives on the Individual Treatment Plan.
  
• Account balance will be paid in full.
  

Unsuccessful Completion

• Client withdraws from treatment.
  
• Client refuses to participate in treatment.
  
• Client is unable to meet financial obligations.
  
• Client violates code of conduct.
  
• No contact (abandonment of treatment) within a 30-day period.
  
• Client incarcerated greater than 30 days without notification.
  
• Client transfers to a more intense level of care.
